Abstract: Schwannoma is a benign tumor arising from schwann cells. Schwannoma arises frequently from the acoustic nerve. In the peripheral nervous system usually found in relation to main nerves of the limbs like flexor aspect near the elbow, wrist or knee or the head and neck. Perineal and scrotal schwannoma is very rare. We report the rare case of 55-year-old man presented with a 4-5 years history of multiple swelling over perineum and right hemiscrotum. MRI reported multiple enhancing solid cystic component post contrast enhancing lesions in perineal region. The clinical suspicion was a skin appendageal tumor or benign lipomatous tumor. Patient underwent a complete excision of the tumor. The histological features were suggestive of perineo-scrotal schwannoma.
