Neutrophil to lymphocyte ratio as a predictor of amputation in patients with diabetic foot ulcer at RSUP Dr. M. Djamil Padang

Background: Diabetic foot ulcer is one of the most important and serious chronic complications of diabetic patients with severe consequences leading to amputation and even death. The Neutrophil to Lymphocyte Ratio (NLR) is the new biomarker that serves as a inflammation-based predictor value in many disease. NLR are inexpensive and easy to access predictor parameters in the diagnosis and follow-up of diabetic foot ulcers. The aim of this study was to investigate the predictor values of neutrophil-to-lymphocyte ratio in diabetic foot ulcers treatment.
Method: This study was a diagnostic predictive diagnostic study with retrospective design to determine the influence of NLR value as a predictor of amputation in patients with diabetic foot ulcer. Data were conducted consecutively by medical records of patients with diabetic foot ulcer at RSUP Dr. M. Djamil Padang in November 2022 - September 2024. An analysis data with a ROC curve to determine the best cut-off point, then the data was analyzed in 2x2 table to determine sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV). All the data was analyze using SPSS 30.0.
Results: A total 50 patients of diabetic foot ulcer were included in this study. The age of the respondents in this study had a median value of 54, 36 years old. Male respondent were 27 people (54%) and female were 23 people (46%). Respondents underwent hospitalization with a median of 11, 26 days. The median result of haemoglobin examination was 9.13 g / dl, leukocytes were 18.224 103/ ml, the neutrophil count were 79, 64%, the lymphocyte count were 12, 62%, the NLR were 10,44, albumins were 2,49 g/dL, and blood glucose level were 231,62 /dL. The ROC analysis showed NLR cut-off point value was 7, 82 with AUC value was 0,908 (CI 95% 0,822-0,989) with a sensitivity 85%, specificity 86%, PPV 81% and NPV 89%.
Conclusion: The Neutrophil to Lymphocyte Ratio are reliable prognostic factors in patients with diabetic foot ulcer.
